Enrollment Paperwork


Welcome to our school's enrollment page! We are thrilled you are considering our school for your child's education. Enrolling your child in school is an important decision, and we want to make the process as easy and seamless as possible.

Our school offers a comprehensive and challenging curriculum that is designed to prepare students for success in High School and beyond. Our dedicated and experienced teachers work diligently to create a supportive and inclusive learning environment where students can thrive.

To get started with the enrollment process, fill out our online enrollment form or contact our office, Deanne Coleman, for assistance. We are happy to answer any questions you may have and provide you with the information you need. Please review the steps to ensure your enrollment is smooth, as they will guide you through the process.

*Please note that we will not process your paperwork if you have not been accepted or placed on the 2023-2024 school year waitlist.

STEP 1: Please click the link to open the CPMA Enrollment Packet. The link will take you to PDF fillable forms. 


STEP 2: Send Proof of Residency to our school clerk, Deanne Coleman, at [email protected], or return a copy in person to our school site. Please note that the parent/guardian must submit Proof of Residency to process your paperwork. 


STEP 3: Please click the link to open the Course Request Form (Arts Pathways). The link will take you to a form to fill out the information about the arts pathway your student is interested in. 


STEP 4: Please click the link to open the Transportation Application. The link will take you to a Google Form where you will fill out the information to apply for a bus for your student.
